In the alphabet soup of acronyms, nothing quite compares to the patient notification forms. There's the IMM, the MOON, HINNs, and ABNs. No wonder there's confusion. And when there's confusion, auditors are poised to pounce.
In fact, nothing gets the government's attention faster than a complaint about violating a beneficiary's rights. While hospitals have been required to provide the Important Message from Medicare (IMM) for the last 10 years, many still have not mastered the process. The controversy over observation led Congress to pass the NOTICE Act requiring written notification to observation patients and, subsequently, the Centers for Medicare & Medicaid Services (CMS) created the Medicare Outpatient Observation Notice (MOON). Yet, today, many hospitals are still confused when and where to use the Advance Beneficiary Notice (ABN) and how to use the many Hospital-Issued Notice of Noncoverage (HINN) notices.

If a patient lodges a quality complaint or appeals a discharge and the form is not completed properly, it could result in a survey by the accreditation agency, a fine, or inability to collect revenue. It also produces negative patient impressions.

When patients complain or appeal, the first thing that the QIO reviews is the patient notification form. If that is completed improperly, the case is decided in favor of the patient, no matter the clinical circumstances. Providing these forms properly is a requirement and often the duties are distributed to many departments throughout the hospital. Because improper form distribution or completion could result in significant compliance risks, including a citation from an accreditation agency, getting it right every time not optional.
